Zepbound vs Wegovy

How do Zepbound vs. Wegovy Compare?

GLP-1 medications are now a common tool in medical weight loss programs. They may help control hunger while you work on enhancing your nutrition, increase your physical activity, and make other healthy changes. Two widely discussed options are Zepbound and Wegovy. Both support long-term weight management, but they are not the same. Zepbound contains tirzepatide and targets your GIP and GLP-1 hormone receptors. Wegovy contains semaglutide and targets your GLP-1 hormone receptors.

How Do Zepbound and Wegovy Work?

Your body makes and releases GLP-1 and GIP after you eat. These hormones send messages between your gut and brain. They help control hunger and blood sugar. They also help you feel full and slow how quickly food leaves your stomach. Your body does not keep these natural signals active for long.

Wegovy and Zepbound use the same hormone pathways to keep those signals active longer. Wegovy acts on GLP-1. Zepbound acts on both GLP-1 and GIP. This may help you feel full sooner and stay full longer, though results and side effects vary.

Wegovy vs. Zepbound Weight Loss Results

The best direct evidence comes from the 72-week SURMOUNT-5 trial. It included 751 adults with obesity who did not have type 2 diabetes. They received the highest dose they could tolerate of tirzepatide or semaglutide.

Average weight loss was 20.2% with tirzepatide and 13.7% with semaglutide. Eli Lilly, Zepbound’s maker, funded the study. The trial tested tirzepatide 10 mg or 15 mg against semaglutide 1.7 mg or 2.4 mg. It did not test the newer Wegovy HD 7.2 mg dose.

In separate trials, average weight loss was 14.9% with semaglutide 2.4 mg and up to 20.9% with tirzepatide 15 mg. The studies used different plans and time frames, so they are not a direct comparison. Your results may be higher or lower.

Eligibility and Approved Uses

For adult weight management, both drugs use the same main BMI limits. You may qualify with a BMI of 30 or higher. You may also qualify with a BMI of 27 or higher and a related condition, such as high blood pressure. Meeting the BMI requirements does not mean either drug is automatically right for you.

The latest FDA labels show other differences. Zepbound can treat moderate to severe sleep apnea in adults with obesity. Wegovy has added uses for certain people with heart disease or metabolic dysfunction-associated steatohepatitis (MASH), which is an advanced form of fatty liver disease where fat buildup leads to dangerous swelling and permanent liver scarring. Your doctor will review your age, health, and reason for treatment.

Mounjaro and Zepbound both contain tirzepatide. Ozempic and Wegovy both contain semaglutide. Each brand has its own FDA-approved uses, doses, and label, and one is not an automatic substitute for another.

Doses and Injection Devices: Zepbound vs. Wegovy

Both injections start low and rise in steps to help limit the common stomach side effects. Zepbound starts at 2.5 mg weekly. Wegovy injections start at 0.25 mg weekly. These dose amounts are not equal.

This is not a Wegovy vs. Zepbound dosage chart for self-treatment. There is no approved dose conversion. Your doctor will choose the pace and maintenance dose based on your response and side effects.

Zepbound may come in single-dose pens or vials, multi-dose vials, or a four-dose KwikPen. Wegovy injections come in single-dose pens, including Wegovy HD. Your administration device can depend on your prescription, pharmacy, coverage, and program.

Side Effects, Warnings, and Monitoring

Common side effects are mostly digestive. They include nausea, diarrhea, vomiting, constipation, stomach pain, indigestion, and reflux. Zepbound may also cause injection-site reactions. Wegovy HD may cause skin sensitivity or burning.

Both labels have a boxed warning about thyroid C-cell tumors seen in animals. Do not use either drug if you or a family member has had medullary thyroid carcinoma or if you have MEN 2. A serious allergy to the drug is also a reason not to use it.

Other risks include pancreatitis, gallbladder problems, kidney injury from dehydration, severe stomach problems, low blood sugar with some diabetes drugs, and serious allergic reactions. Severe or lasting stomach pain, dehydration, or signs of a serious allergy need prompt medical care.

Your care team may track your weight, side effects, kidney health, blood sugar, and eye health. Both labels warn of possible harm during pregnancy. Your doctor needs to know about your drugs, supplements, planned procedures, and pregnancy or birth control concerns.

Zepbound and Wegovy should not be used together. They should not be combined with another GLP-1 medication or switched without medical guidance.

Zepbound vs. Wegovy Cost and Coverage

Prices and coverage can vary widely. Some private insurance plans now cover Zepbound, Wegovy, or both, but each plan has its own rules. Coverage may depend on why your doctor prescribed it, whether the drug is on the plan’s formulary, and whether prior authorization is required. Deductibles, copays, and coinsurance can also change what you pay.

Eligible patients with commercial insurance may qualify for manufacturer savings cards or coupons. Others may find lower cash prices through direct-pay or pharmacy discount programs. Each option has its own rules and expiration dates. Manufacturer savings cards often exclude people with Medicare, Medicaid, or other government insurance.

Dose strength matters, too. Some programs charge the same across several doses, while others charge more for higher strengths or different devices. Review IBI Wellness’s current affordability update and pricing page, then ask your insurer and pharmacy to confirm your cost.

Which Is Better, Wegovy or Zepbound?

Zepbound led to more average weight loss in the direct trial against Wegovy 1.7 mg or 2.4 mg. Wegovy may fit better based on its other approved uses, your health, side effects, device choice, coverage, or supply.

The best choice is one that fits your needs and circumstances, is easier for you to tolerate and access, and includes your follow-up care. Book a consultation with IBI Wellness to discuss which options may fit your needs.
